Community Mental and Physical Health Support.
Hours per week: 37.5 hours per week
Summary of main duties and responsibilities:
To work as part of the multidisciplinary team.
Contribute to assessment of both physical and mental health needs. Focusing on the wellbeing of those with serious mental illness, and demonstrating an understanding of the relationship between physical and mental wellbeing
To work with other agencies to deliver a systems approach in ensuring patients mental and physical health wellbeing needs are met.
To provide information on health promotion including healthy eating, sport and exercise and smoking cessation.
To support patients to consider all needs and identify their own physical health goals, and to support monitoring goals identified with other members of the team.
Rates up to £15 per hour.
